| Options For Independence: TERREBONNE-LAFOURCHE
RESETTLEMENT SERVICES |
I. Program Description: The agency will provide community based Resettlement
Services designed to aid consumers with serious mental illness to resettle
into their respective home communities following hospitalization, extensive
community alienation or homelessness due to mental illness.
II. Program Services: Services in the Terrebonne-Lafourche Resettlement
Program include:
- A. Transportation to/from service providers, such as community
mental health providers and other agencies providing services related to
resettlement.
- B. Basic supportive resettlement counseling for consumers and
family members.
- C. Development of a support system through notification and application
of resettlement to agencies and supports such as:
- Community Mental Health Centers
- Rehabilitation agencies
- Drug and alcohol agencies
- Entitlement and financial support programs
- Housing and supported housing programs
- Legal, law enforcement and judicial programs
- Transportation programs
- Utility programs
- Family support and consumer support programs
- Landlords, neighbors and other natural support systems
- D. Basic medication training and compliance program
- E. Assisting family, friends and others to develop a natural
support system to respond to resettlement needs/issues
- F. Emergency housing services for individuals identified as homeless
or at risk of homelessness due to their mental illness, these services
may include:
- Transportation from sites of need to emergency housing sites
- Community health provider sites and other agency sites required
for emergency resettlement
- Referral and access and use of DHH/OMH Consumer Resources as needed
for food and
- Clothing, housing, medicine, etc.
- G. Provide 24 hour access to emergency services when needed.
