Program Description: McKinney Outreach provides transitional housing
and supportive services to the homeless living in Terrebonne and Lafourche parishes,
which aids them in moving out of homelessness and into self-sufficiency and stable,
permanent housing.
II.
Program Objectives: McKinney Outreach will provide outreach and assessment
services to the homeless living in Terrebonne and Lafourche parishes found through
street outreach as well as through referrals from local social/community service
agencies. Outreach workers will conduct an assessment to determine eligibility
for program participation (as dictated by HUD) and develop an action plan aimed
at assisting the individual to achieve permanent housing. The program nurse will
also conduct a health assessment. If deemed eligible, the consumer will be referred
to case manager within the project.
